Physical Therapy – Green & Moncrief

One of the certainties of competitive soccer is that from time to time, players will get hurt. When an injury occurs, it is important to have an early diagnosis and to start appropriate treatment as soon as possible.

In order to facilitate this process, Green & Moncrief Physical Therapy (link to About Green & Moncrief below) has offered to provide a free diagnosis and treatment recommendation for any member of the Saddleback United family. This is an optional service and there is no obligation to seek or continue treatment with Green & Moncrief, but it is highly recommended.

In the year plus that Green & Moncrief has been seeing Saddleback members, the results have been exceptional.

Here is how the program works…

1. As soon as possible after an injury occurs, call the offices of Green & Moncrief at (949) 582-8800 to schedule an appointment. Be sure to tell them that you are with Saddleback United!!!
2. At the appointment, you will be asked to fill out an optional release of information form. This form authorizes the doctor to share the injury report information with the Saddleback United coaching staff (Sample Injury Report Form). This is optional, but highly recommended and the information is only shared with coaches that need to know and is treated as confidential.
3. The doctor will evaluate and diagnose the injury and provide treatment recommendations.
4. If authorized, the doctor will fax a copy of the Injury Report to Saddleback United.

There is no charge for the first visit and if you decide to seek additional treatment from Green & Moncrief, their fees are reasonable and they take most insurance plans.
